The Type of Information Involved In a Personal Injury Claim

Every personal injury case is different. Information may be requested from time to time, depending on the case. However, the seven items below typically represent what is needed to properly document your case.

  • You'll need to completely fill out the forms that accompany Matt Napier's initial letter to you and return them to us.

  • You'll need to periodically send us written updates summarizing your injuries, treatment and progress (ie. monthly, during the preliminary recovery period, and then every 1-4 months as injuries heal and improve, etc.)

  • We will need medical reports and records from your family doctor, along with those of any specialists and therapists - i.e., physiotherapists, chiropractors, etc.) who assess and/or monitor your condition and treatment. (We usually obtain these directly from the medical care-givers as soon as you have signed your medical authorization forms, along with updates as your treatment progresses.)

  • We will need documentation supporting any lost wages you may have incurred as a result of the accident. In order to do this, you will have to get a form completed by your employer (the Employer Confirmation of Income and Benefits form).

  • You may need provide copies of your income tax returns for a specified period of time. (We'll ask you to obtain these, if needed.)

  • We will need the invoices/receipts for any out-of-pocket expenses that you may incur as a result of the accident. Specifically: expenses that are not covered by your medical insurance company (i.e., Blue Cross etc.) or as part of your Section B claim. For more information on Section B, please review our FAQs.
